Is there anyway to have these just control iTunes? Im sure that used to be a thing (default) in Sierra.
Now if I'm surfing the web whilst listening to iTunes and then start a video I have to open iTunes to pause it again.

I realized I googled the wrong terms. I thought the keys not working was a bug. When you google High Sierra Media Keys you realize through dozens of posts and websites this is how Apple programed the keys to function now and left no preference to adjust. For example natural scroll which I cannot stand.
