In portrait mode on the XS Max (probably applies equally well to XS and X, possibly 8+ and 7+), which lens is the default lens?

I am specifically wanting to know if it goes to the telephoto lens right away. The reason is that I'd like to maintain 12 MP shots regardless of whether the lens blur effect is available. For example, if it's too dark or the subject is not within the necessary distance, I want to still have a 12 MP shot when I hit the camera button.

Is that the case? Or is the wide angle the primary lens, and the resultant photo is less than 12 MP (i.e. a cropped photo from the wide angle lens)?

Portrait mode seems to go to the telephoto lens by default. You can check this by covering the bottom lens with a business card or similar. I don’t think you can force portrait mode with the other lens, but I could be wrong.
