I have a friend that wants to buy the Apple TV 4K and does not have any Apple devices or any Apple ID. Is she able to activate the Apple TV 4K and use it as normal. Is she able to download apps etc via app store on her apple tv 4k?

Even if you skip that it’ll prompt to do so as soon as you attempt to download an app.

Yes.
I bought an Apple TV 4K 3rd gen and I have no other Apple devices or Apple ID.
On initial setup you can create an Apple ID as part of the process. My Apple ID is now my Hotmail email address 👍
