If I leave my phone with Do Not Disturb mode on, calls coming in won't be forwarded to the watch. I've noticed this behavior on Series 1 and also Series 1 LTE.

Changing the phone notifications to mirrored/custom makes no difference, and the Watch is not on Do Not Disturb mode. Does anyone have a solution? You can pretty easily replicate this by using another phone to call and test.

You must have a good reason for your phone to be on DND.
Can’t you just silence it?
If the phone is locked, the watch should take all notifications, if set correctly.
If phone is unlocked and silenced, then it shouldn’t make any sounds. Again, if it’s set up correctly.
That’s what I do.

On a similar note though, I wish there was a way for all notifications to go to the watch. I hate it going to my phone just because it’s unlocked. Unlocked doesn’t mean I’m looking at it, or even nearby. Stupid.
