Is there a way to make a PDF with Preview that will allow you to navigate through it in iBooks?

Links from preview 5.0 seem to be gone so you can't make TOC entries,
Bookmarks made in preview don't open in iBooks.

Do I really need to buy 3rd party software for such a mundane task?
