I set up a new 4s the other on Verizon, also have a iPhone 4 on AT&T and set up using iCloud over the air. The odd problem is that many of my US contacts (i am also in the US) are showing the Netherlands as thir country in the contact screen and no state is shown. I am syncing contacts with exchange server and my IPad and other iPhone show the or correct contact and country info. Did I somehow check something odd in settings that could be causing this? There has to be some simple reason this is happening, any help is greatly appreciated. Thanks

I just moved to Verizon from AT&T, when I restored by contacts from my 3gs to the 4s I experienced the same thing. I found on the net to dial *228 and proceed with option 1 to activate your phone again. It takes about 30 seconds on the phone and about 2 minutes for the contacts to update. After I did that, the (000)000-0000 format were in my contacts. Also, when I went to add a contact, it defaulted the united states instead of the netherlands after the reactivation.

I found this on another site and it worked for me.

Yeah!
