Problems with app icons in iPhone 3.0 and StarDefense

Every time I download a new app, the app icon is of the app I last deleted. Should I restore my iPhone or is this a general bug. Also on StarDefense, Plus+ is not working and saying my first name is invalid. It is me or the app?
 
with a little movement of apps, I believe I fixed my problem. As for Plus+, it is a new service and I am sure ngmoco is fixing it as the days go by.
