Professional Form Creation Application

I need to create some forms for a Home Healthcare business. My main concern is creating neat professional looking forms with check boxes and blank spaces that can be exported to PDF format and distrubuted. A secondary ability would be for these forms to be electronic interactive forms. But again, just being avle to creat some sharp looking neetly formated documents is my primary concern.

Any suggestions?
