Hello,

My proximity sensor is no longer turning off the screen when covered. My phone is jailbroken on 4.0.1 IOS, is it possible that it's related to one of the jailbreak apps and restoring to factory IOS could fix it or is it more likely a hardware issue where something needs to be replaced?

I'm assuming you've rebooted the phone. I seen similar software issue, so I don't think that it's hardware. Restore your phone back to stock 4.0.1 and see if you have the same problem.
BTW - I would suggest the same thing, even if you were not JB.

Update: Proximity sensor replacement cable (4.00 shipped on ebay) fixed my problem. Pretty easy to replace if you're the slightest bit handy. :D
