When on the beta site, I go to enroll iOS device, and when I click install profile, it asks me if I want iPhone or watch. I have iOS 12 already on the phone so I chose watch. Beta profile got installed on the watch and my phone and watch rebooted. When I check for an update, it says I’m on latest 4.3.2.

What am I doing wrong? Thanks.

There is no public beta for watchOS 5, the beta profile is for iPhone only and will do nothing even if you have it installed on the watch. if you want to install watchOS 5 beta, you need to have either a developer account or a way to get the corresponding profile (google it, you will find it easily, but use it at your own risk).
 
What’s the big downside of doing a beta test? Does it lock you into having to always upgrade to the latest beta release? For example if I do this then down the road want to stop can I?
