Carrier Purchase replacement Apple SIM

I can't find this online anywhere, but where do you buy a replacement Apple SIM? Is it only in the Apple store itself, or can you do it online somewhere?

I purchased a couple at the Apple store for $4.99 each. I don't believe you can purchase them anywhere but an Apple store.

Does it work in the iPhone in the same way?

I am on EE in the UK and when I'm in Spain I would like to select a local carrier there without having to switch SIM cards every month.

Only works in the iPad Air 2 and iPad Mini 3. If you place in another IOS device you get a SIM error.
