does an iPhone's online billing statement show my activity while I'm just on wifi, just using cellular data or both? Does it show which apps I've downloaded? does it show my activity within those apps? Is my search history available to the holder of my online billing statement if I'm using an external app like google's search app? If I'm using Skype or another external app to make calls and texts, are those available to see on my online billing statement?

It shows carrier data usage. Not wifi since that's not measured or billed by your carrier.
It does not show what sites you visited or apps you used or downloaded, just amount of carrier data used per day.
It does show phone numbers you called, what time you called and for how long. Also shows texts message send to other phones but not the content of the message. imessages dont show up on the carrier phone bill.

Now that all being true, most cellular providers do have parental controls (some free and others not) that can be implemented to monitor or restrict usage times, downloads, excessive use of data, etc.
 
Verizon shows data utilization per line in categories such as: web & apps, video, maps & navigation, social media & networking, audio, gaming, email & messaging.

Isn't that something you'd also need access to the phone itself to implement? Or are those parental controls set up without knowledge of the person with the phone?

To my knowledge, it depends on the controls. Some are on the phone, while Produver Prental Controls are all done online via the cellular provider's website and within the bill-payer's login. I could be wrong though.
