Question about Streaming Flash Video in Dreamweaver

So, I'm trying to add a streaming video on my page using Dreamweaver. Under Insert > Media > Flash Video, what do I enter under "Server URL: rtmp://"?

It's asking me to enter rtmp://server_name/app_name/instance_name

server_name would be www.mydomain.com. But what are the other two??
