Quick question about iphone upgrade program

Assuming I want to exchange my iPhone 6s+ after one year on the program, what exactly am I required to bring to give back to Apple to follow through with the one year upgrade. Obviously I will give them the phone back but does it need to be in its original packaging? Do I need to give them the ear buds and the charger that came with the phone?
