Quicky Reply on Skype(other apps?) Major bug

Ok see attached screenshots. iPhone 6+ 64GB, iOS 9.0.2. This is happening every time I use quickreply for the new Skype only from the lock screen.

This does not seem to happen when quick replying to skype message when phone is unlocked. only way this ghost keyboard to go away is to do a pull down for Spotlight search onthe home screen.

Anyone else seen this?

Where do I report this to Apple?

It's a know issue and has been fixed in the latest 9.1 beta. As it stands, due to this issue, quick reply is fairly useless on 9.0.2.
