reinstall itunes AGAIN...

Several times now I've had to reinstall iTunes to get my iPhone or any iOS device (other than my old 160GB AppleTV) to be recognized in iTunes. I want to say this is the 5th time I've done it now. Any ideas on what might be going wrong and how to fix it?
