I want to reinstall OS X in my main drive from OS X disc but want I to restore my music & photos from Time Machine. Do I perform the reinstall from disc when it asks? Then after everything is done (up and running) plug in the Time Machine drive to restore iTunes muisc folders and photos from iPhotos?

-Which folder in iTunes and iPhotos? Or do I restore the app too?
 
Frankly for what you're doing I'd just suggest doing an archive install rather than messing with a reformat reinstall. This way you save your user and get rid of all the extensions or other buggy system files that may be causing you problems.
 
I would suggest the Archive and Install. Otherwise you need to set up Time Machine after installation and then select the files/folders you want to restore individually.
