Relink Audio Device After Wake From Sleep

I think that the title is pretty much self-explanatory. I am looking for an efficient way to relink my audio device (us-144), after my mac mini wakes from sleep.

Is there an Applescript that can be run afterwards and relink it? Essentially what i do each time is go to system preferences and change the audio output to built in and then change it back to us-144 for the audio driver to kick in.
