Replacing the cam button on lock screen

If I want to use camera + as my default camera, is there any way to use that instead of going to the cam app when I unlock.

I don't think so. When using the camera app, the camera roll is not viewable, only those photos taken during the current session launched from the lock screen. I doubt third party camera apps have this kind of feature so you are asking for a gaping hole in phone security which Apple is not about to grant given their need to make iPhone palatable to corporate users who rely on the absolute integrity of the phone lock screen.
