Replies going to wrong email address

I've seen a few threads similar to this, but the fixes aren't applicable to my situation.

I have three accounts set up in Mail...

1) personal gmail account
2) website domain email
3) work email (exchange server)

Today this happened twice... I'll send an email from my work email, and it will show up in my sent box under my work email with my work email in the from box. The person will reply to my email and it will reply to my gmail account. I've checked the outgoing mail servers and they're all correct (gmail outgoing for gmail account, exchange server for work account).

Any ideas?
