Resolved: How to prevent the Iphone from being listed twice under the Text Forwarding option

When I have done a clean install of OS X in the past, my iPhone is listed twice under the text forwarding option. I guess I must be missing a step in signing out on the Mac and the iPhone, in order to prevent this from happening.

Can someone list the correct steps, so that (hopefully) I don't run into this problem again, when I do a clean install of Sierra?
