I purchased an iPhone 6 Plus using one of my Verizon upgrades. It was preordered on the 12th, and arrived on the 19th. I believe I activated it on the 19th.

I need to return the phone, and if I return it to Apple, does my Verizon upgrade automatically go back to my account? Is the return period for Verizon 14days from activation, or form order date (9/12)?

I just went through the same thing with ATT. Apple just got my NEXT PLAN device today (under the 2 weeks). I called AT&T and they told me "it should and if not to call." I was already billed and paid for for my first NEXT payment so I'm giving it a week or so before bugging them since my next NEXT :) bill isn't for a month.
 
Update ... I shipped the phone back to Apple, they received it at 10AM this morning, and I already have a credit on my ATT bill and my "NEXT" eligibility has been updated to "available" for that line so that should answer your question.

Apple is fast on posting returns.
