Safari AutoFill Pop-Up Window Stuck

I am currently running Mac OS 10.8.5 with Safari 6.1 on an iMac.

When I attempt to AutoFill a web form, the blue pop-up up window appears ("AutoFill your contact info…"). When I click on it, nothing happens. The AutoFill pop-up window remains but none of the fields are filled.

When I click on another field on the same web page, the initial AutoFill pop-up window remains but when I attempt to AutoFill another field, another AutoFill pop-up window appears with the same behavior as before.

If I refresh the web page, the AutoFill pop-up windows disappear.

How do I get Safari's AutoFill function to work again?
