Safari Error Message

I get this error message when attempting to use AutoFill
for signing into online accounts such as Amazon. I know
that AutoFill uses information from Address Book to fill
out the form - that's why I have a card in Address Book
with my name and email address on it - but I'm still
getting this error message when attempting to use AutoFill.

Have you reset Safari yet or taken a look at Safari > Preferences > AutoFill?

Yes sir, I've reset Safari several times since this problem occurred. I don't like
keeping a long history anyway. I pretty much reset the browser I'm using everyday.

What am I looking for?
