Safari Fav icons keep changing

The fav icons in Safari bookmarks occasionally change to icons from other sites and while it's not that big of a deal it is annoying. I can delete the icon and add it back again to fix the situation but what is causing this to happen in the first place?

I was wondering the same thing, but least it is only one (Outlook) that does it with me.

Happy to report that the iOS7.1 update fixed the issue. At least the icons that were messed up reverted to the proper icons after the update. So fingers crossed and hope this really is a permanent fix!
