Safari - Not Saving Login & Passwords

Does anyone notice or have a fix for Safari not saving logins or passwords for websites. It seems to reset all my saved login and passwords every couple days. Must be a bug? Very annoying one.

When you enter your username and password, try hitting Refresh, then log in. That's what I have to do to receive the prompt to save my username/password. Annoying, but works.
