Generally can re-create it pretty consistently by opening a tab and entering a URL. Now open a new tab, enter another URL. Click back on other tab.

It will replace the 2nd URL into the first tab, and make the 2nd tab blank.

If you are on very fast internet it probably won't work because the key seems to be the pages have to be loading. However it doesn't take long to open new tabs with URLs when using keyboard shortcuts, only 2-3 seconds for the whole process.

This is very common every morning when I open a bunch of sites.

Curious to know if anyone else has had this problem. It's been happening for maybe a year for me.
