I’m using the YouTube web applet (not the app) in Safari.
The problem is when I go to settings and turn off Video Previews, the off selection only sticks for a short amount of time. Sometimes it stays unchecked for an hour up to a 6-7 hours. I haven’t figured out what’s making the settings go back to default.
At first I thought it was my ad blocker (1Blocker) so I deleted it to see if that helped. It didn’t.
If I clear off all safari pages and force quit the app the settings don’t necessarily revert to default (preview on).
What would be clearing my selection?
 
Same annoying problem here still a thing in May. Setting used to remember. I don't have any special settings and I have content blockers disabled on YouTube since I use Premium anyway.
 
I was having this issue as well, both on my Mac and iPhone, though I use plain Safari, not a webapp. I think I solved it, it's been several days and the settings have not been reset.

What I did is go to Safari -> Settings -> Privacy -> Manage Website Data (macOS) and Settings -> Apps -> Safari -> Advanced -> Website Data (iOS). In there I searched for youtube and google and deleted everything that came up. I remember seeing an odd one, something about no-cookie or similar, I should have taken a screenshot. After that, I quit Safari, re-launch it and loaded YouTube and signed in. Then I set my settings and so far so good.

You could also just delete everything, but I do have a bunch of other sessions and settings for other sites that I'm too lazy to set again.
