My iPhone now looks like the attached photo. Any idea what might be wrong and/or what I might need to replace?

Yep, looks like screen needs a replacement.
Find a local iphone repair place or bring it to Apple.
If you're under warranty they will do it for free, if not they will charge you a fee according to your iphone model.

To clarify, Applejuiced means that your LCD needs to be replaced. Based on your model of iPhone, that could include the glass as well.

Yes, not just 1 piece, usually its 3 pieces fused in together like glass, lcd and digitizer.
