I plan to sell my iPhone 4S when the 5 comes out. If I do a factory restore and wipe the phone, does this wipe the information from the SIM card? How do I get my personal info off the SIM card before I sell the phone, or should I just sell it without the Sim?

Definitely no SIM! Your phone numbers, data etc could be stored on that SIM so destroy if you're not going to reuse it.
 
Unless you've used another device that actually stored data on the SIM there probably isn't anything on the SIM. That said, there's no reason to provide your used SIM as it is (or was) tied to your cell account.
