Send music from Mac to iPhone NOT on same network?

I would like to start playing an iTunes playlist my home computer, leave that computer at my house, and while I am away from the house - NOT on the same network - stream that audio from the Mac to my iPhone. Is there an iOS app (and/or a Mac app) that will do that? Thanks.

I don't think Home Sharing or Airfoil will work because they appear to require that I be on the same network.
