This has been bugging me since I got my iphone: Is it required that I
have 2 separate accounts for (1) my iphone, ($70/month) (2) the other 3 ATT phones for my family ($100/month). Is it possible to COMBINE them all into
one account?

Don't see why you should have separate accounts. In my family, we had 1 iPhone and 2 other phones and now we have 2 iPhones and 1 other phone.

Do you mean one rate plan as in a FamilyTalk rate plan? The answer is Yes but you will have to share the minutes across all 4 phone lines. So you can combine the lines under a single account and have a FamilyTalk plan with all 4 lines. Only the iPhone will requre the iPhone data plan. There is really nothing special other than the data plan.

The iPhone 3G can be the primary line on the FamilyTalk rate plan or one of the add-a-line(s).
