I had my Mac repaired a couple of months, and just today I just realized OS X doesn't know my serial number. See attached screenshots for more information. Thanks.

It's fairly common after certain repairs, especially logic board repairs. Take it back to Apple and they can restore your serial number.
 
If it bothers you they can fix it fairly easily. You just need to bring it into them
 
I had the same situation when I had a board replaced. Took it back to the authorized Apple repair shop and they serialized it in under 10 minutes.
