I have Apple Watch S0 and S3 both running latest watchOS.
I barely use S0 these days, but when I switch this battery problem happens to me.

And I have been experiencing a drastically battery drain if I switch to use another paired Apple Watch in some occasions.
This will only happen if there's any update for 3rd party apps already installed on iPhone.
After switching those apps will try to update to Apple watch, but it never did. The update circle will keep rolling for hours even for only 1 app update.
Then the watch will last only 4-5 hours for S0, and around 6-8 hours for S3.

Here's the scenario

Current connection : iPhone + AW S3 with 5 3rd party apps installed
If I update any 3rd party apps while connecting to S3, it will also update on my S3 just fine.
Let's say 3 days later I decided to use my S0 watch, after switching I can notice those apps update are spinning on "Watch App", but it never succeed, and the watch battery will last for 4-5 hours.
The only way to solve problem is to turn on and off Airplane mode on my S0 until I see those apps are actually has been updated, then I'll have a normal battery life.

It works the other way around, if I update any 3rd party apps when my S0 is a current watch, later when I decide to switch to S3, the apps on S3 are not going to get updated until I restart the watch or turn on and off the Airplane mode.

This problem never happen if there's no updates available on any 3rd party apps.
I tried to report to Apple several times over a year but this problem still occurs to me.
PS. Already tried several unpairs, re-pairings, restoring iPhone, set up as a new watch, set up as a new iPhone. Still, didn't help.
