Sometimes after I do a Hard Reset "Hold Sleep and Home Button till iPhone Turns Off" when I turn my iPhone back on sometimes the progress bar comes up and sometimes it doesn't but sometimes my settings are changed from what I had them. The screen brightness, % indicator is off, Notifications are on, somethings in sound are different, along with a few more settings.

This doesn't happen all the time, It seems to be after I install or delete apps. If I do it just randomly after using my iPhone all day it doesn't.

Is there an issuse with my iPhone?

Also sometimes when I hold the sleep down and then swipe to turn off it starts to turn off but then comes right back on, is this an issue also?

This happened to me a couple weeks ago. But get this, it was on the iPhone 4 and iOS 5 beta 7. Exactly as you described it. Same settings effected.
 
Same here, I've noticed it 3x so far ... 16GB iPhone 4S from Verizon on stock iOS 5.
 
I restored as a new phone and reinstalled everything clean, problem went away.
