Can anyone recommend an app that allows two separate iPhone users to collaborate on one note or to do list? I wish this was for something fancy, but basically I'm looking to have a real time shared list that my wife and I can both update with groceries, etc, without having to text back and forth all the time.

Does this feature exist in the notes app that comes with ios6?

Use the Reminders.app, which you can share with your wife:
