Shortcuts widget can't open apps from lock screen

I created a shortcut that opens an app. Every time I run it using the Shortcuts widget from the lock screen, I get an error saying "Open App failed because the app could not be opened". It works fine if I unlock the phone and go to the home screen first before swiping to the widget screen. However, if I simply unlock the phone, but stay on the lock screen and swipe straight to the widgets from there, it returns that error.


Any idea what's going on? This just started happened in Shortcuts 2.1 both before and after updating to iOS 12.1. It's also doing the same thing on my iPad.
