I got this T5 SSD to be used with my Macbook Pro as my macbook only has 128gb SSD and i can't store my music library on it or anything of large size. My Macbook Pro 2017 Model is currently on MacOS Catalina. Should i format the new T5 to APFS or MacOS Extended? im basically going to have the T5 connected to my macbook at most if not, All times. I'll be having all my documents on the T5, My entire music library on it, iPhone Backups through the music app etc. Basically will be using it to store my data as the built in 128gb of my macbook isn't enough to do much.

Also, can TRIM be enabled for the T5 and/or does it come with TRIM enabled out of the box?

The drive format would be your choice. I don't think there is a great advantage to the APFS format on a non-booting drive, plus, if you ever need to connect that to an older Mac system, El Capitan and older won't be able to access an APFS volume.
Enable Trim manually on your Mac with a terminal command:

sudo trimforce enable
Your Mac will force a reboot after that command
I am pretty sure that trim will not be enabled for a USB drive. If you decide to "upgrade" to a thunderbolt external, macOS will support trim on that.
(TRIM is not particularly important on a non-booting external drive, IMHO.)

The T5 I just purchased is not a boot drive, my boot drive is the internal 128gb SSD in my 2017 MacBook Pro. This Samsung T5 is a drive that I will have connected to my MacBook at all times and will have all my media and documents on it such as my iTunes Library etc

Trim: The T5 is trim capable but afaik Apple haven't enabled trim via USB for external UASP drives, so it won't trim the T5. Trim via UASP USB *is* enabled under windows & Linux.
