I use my iphone with two phone numbers (I forward my at&t business cellphone to it) and it works great (displays incoming numbers correctly, etc) other than when I call someone back they now have my personal cell number. It would be immensely useful (and simple?) to have an app on the main screen that toggles the "show my caller ID" preference. Is this possible under the SDK ?

I recall when I jailbroke 1.3 or so that I created a Prefs: url for a contact. I was surprised that typing that into safari did not bring up the pane. I thought something along those lines could be done.
 
Jailbreak apps and iPhone SDK apps are completely different. Jailbreak apps can access any part of the iPhone while the SDK ones can only access what Apple allows it to which is not much. With the current SDK you can not do what you requested.
