I managed to get a replacement from the Genius Bar, they asked me to sign a form with my initials and signature etc, also giving me a copy.

However, when I look at the copy it shows what my problem with my iPhone was and has a section called Repair Estimate stating my iPhone and that the total amount due is £139?

Who is this amount due to? For me or to Apple?
 
It's how much it costs to repair the device, which they pay. If they didn't ask you for any money, you don't have to worry about it. It's covered by the warranty.
 
Mine was similar but toward the bottom there was a total that said zero.
 
Got my ear phones replaced today with no fuss. They remote was sticking and has been since I got it - just never got round to going in.
