Software to monitor network usage by app

Does anyone know of an app that will display network usage by application?

I'm looking for an app that breaks down current network connections by application.

The output would look something like:
Safari.app
1. 15.25.66.33 12.5 KB/sec IN 0 KB/sec OUT
2. 85.65.32.123 55 KB/sec IN 0 KB/sec OUT

Mail.app
1. 15.25.66.33 12.5 KB/sec IN 0 KB/sec OUT
2. 85.65.32.123 55 KB/sec IN 0 KB/sec OUT

All of the apps I've seen don't relate open connections to applications.

Thanks for any suggestions!
