My Apple TV app shows someone watching an Apple TV+ show in my viewing history. It is the show Tehran and a few days ago it showed someone completed watching Episode 1. Today in my viewing history it shows someone completed Episodes 2 and 3. The only other place I have my iCloud login is my iPhone and my Apple TV in the living room. My wife said she did not watch any of these shows. Is there a way for me to figure out what device is streaming my account?

I get that Netflix accounts can get hacked, but if someone has access to my Apple account, it is unlikely since I have dual authentication set up. I also have not had any suspicious activity like someone buying things from iTunes or other apps.
 
Sounds like an OS glitch, or maybe you or your wife accidentally tap the show and then tap the show you meant to the first time.
But oddly enough it shows someone completed all 3 episodes in sequential order in my history. Not just the "up next" but in the completed section.
 
if you click "mark episode as watched" it behaves exactly the same as if you've actually watched the show.
