I have my iMac and my iPhone 4s on one apple id with app store and iTunes.
Here is the problem I bought a new iPhone 4 for my daughter set her up a new apple id and when she go's on the app store my apple id is saved on her iPhone.
There no way I can get it off there that I can see.

Go into the settings apps, scroll to apple store, and logout of your account.
 
If someone signs out of your ID on the iPhone (you or her), your ID shouldn't be able to pop up.

It sounds like you she has apps/music on her phone that were purchased/downloaded from your account. Does this happen if there is update? Or just when she goes to download something?

Also this maybe happening if she is syncing to your iMac and you ate logged in with your id
