Someone stole my keys today and the last seen kept changing locations. Last update was about 2.5 hours ago at an apartment. Is that accurate or is find my guessing where it might be?
 
If you tap on the item in the item list, Find My will usually show a shaded blue circle indicating the approximate accuracy, which varies. Just like all other devices/people in Find My.
 
Update they are extremely accurate.

My keys were travelling around the whole city last night. This morning I get a ping that the keys are at the location I lost them.

I find them outside in a sketchy mans backpack as they were beeping and locked on to him with a 0.1 meter. H threatened to shoot me in the face if I didn’t leave him alone. Eventually the cops recovered them using my phone to find them unfortunately my house keys are the only remaining keys.

Lucky you apparently live in a place where the cops recovered them.
Where I live, San Francisco Bay Area, the cops would not do a thing, as the laws make it very difficult to do so, and the cops would consider keys a relatively trivial thing not worth their time to investigate. All they would do is take a report.

Normally they wouldn’t care either, but I think the threat of shooting me in the face mixed with a hang gun in his backpack is more of what they were hoping to get. They thought they would get him by tracking the keys and fortun I got the keys back in the processes.

If you're close then the precision finding is very accurate.

The Last Seen though isn't necessarily all that accurate since there seems to be a lag between when someone's phone "sees" your AirTag and when it reports its (phone's) location. An Airtag sitting in my garage while I'm away for a couple days will frequently report its location at other houses on my street -- neighbor's phones must pick it up as they go by and by the time their phone checks in with the servers they're down the street.

There's another thread here where someone actively tested it and provided some maps / info on the topic.

Just curious. What keys were they? What would he want with your keys?
