I deleted an app called My Personal Spammer several weeks ago. For some reason this, in the photo is stuck in sys/prefs/login and I have not been able to remove this. What am I missing?

When you remove it from that list, it keeps showing up? Check to see if there are any running processes that are related. If so, hunt them down and delete the actual files. Also look in ~/Library/LaunchAgents.

What is "My Personal Spammer"? Google doesn't help...
 
Spotlight didn't find anything nor did easy find. I did delete this app. Right clicking does not reveal in finder. Mystery.
 
Might be a long shot, and perhaps taking a step back to take two forward, but you could try reinstalling the application, and then using AppCleaner to delete it. That way, you'll know all associated files with the app and be able to delete them.

What do you think?

I used Cleanapp and it didn't find all the files, i had to search for some and delete them.
 
The login items list will show apps whether or not they exist. Uninstalling an app generally doesn't remove items from the list, unless you use an uninstaller app which specifically is coded to remove any of its apps from the list. So if you're deleting an app that launches itself at login, you should check the list manually after deleting/uninstalling the app. But having an item in there that points to a file that no longer exists is not harmful.
