The sound refuses to work with headphones in.

I have:
  • Upgraded OS since
  • Reset PRAM
The sound works fine as normal, playing sound from laptop speakers. Once I put the headphones in the volume bar greys out.

I have held option key right elected the volume in menu.
With headphones not plugged in.

Here is the screen I get with headphones in and when i go to preferences.

Digital out --> means it's sending optical signal, not analogue to your headphones. Can you try with another pair of phones/buds?
 
If the digital output is in use, then you don't get control over the volume through software, so the greyed out volume will be normal for a digital output.
Your audio output socket is not working properly, and there may be a sticking contact inside the port itself.
You COULD try inserting a digital adapter, which reaches further into your headphone port. That will be a toslink-to-minijack adapter.
- Looks something like this one.
Push that in, then pull out a few times. That may reset the connector - or will show you that the port will need to be replaced if you want to use it for any headphones.
If you get to that point of needing repairs, you CAN put that off by using a USB sound card, that will give you a headphone output that you can use until you get your MBPro repaired.
