In my dock, the stacked folders have missing icons (sort of a white paper icon)

All these apps have icons when checked in applications.

Also I have an icon for the stack button itself, and when I add Apps to stack, it changes the main stack icon to blank also.

Try taking the stack off the dock and dragging back on from Finder. If that doesn't work, try repairing permissions...
 
I have done both, but to no avail.

Have no idea what else I can try.

Maybe the dock preferences are corrupted.
Go into hd.-preferences folder and find the dock.plist and trash it; a new one will create auto.

The user preferences folder is in HD/Users/yourusername/Library/Preferences. The OP can then delete the com.apple.dock.plist file, which will reset his dock to all the default settings.

Delete the following:
/Users/yourusername/Library/Preferences/com.apple.desktop.plist
/Users/yourusername/Library/Preferences/com.apple.dock.plist​
Then enter this command in Terminal, followed by pressing enter:
killall Dock​
