I had some troubleshooting to get it to work for me so thought I would share.
1. You do not need bluetooth enabled or paired on either device. Bluetooth is used for other continuity features so enable it on both but no need to pair.
2. You need your iPhone and mac on the same network. Your mac does not require wifi, it can be ethernet only. I have ethernet enabled and wifi disabled and am able to place calls. The mac easily supports having wifi and ethernet at the same time btw.
3. On your iPhone, launch the FaceTime app and sign in. You can close the app once you do this... its a setup step and you never have to use FaceTime on the iPhone again.
4. On your Mac, launch FaceTime and sign in with the same account.
Thats it!
When you receive a call while the iPhone is on the same network, the mac will react. When you click on a phone number from your mac you can launch FaceTime to place a call through the iPhone.
I posted this because I had never signed into FaceTime on the iPhone (it never even occurred to me) and never got the phone feature to work.
Anyone can edit this post to make corrections
