I'm looking for a camera that will take still images while still recording video. Anything I've found has to switch to a "stills" or "screen shot" mode, thus disabling the video recording.

It is for use during sterile surgery and we want to record the whole procedure and take stills as and when necessary. Stopping and starting video to take stills is not ideal.

Preferably, it should have a remote to take the still images but not essential.

Any ideas greatly appreciated. :)

If quality still is what you're after, you'd be better served using a dedicated still camera in addition to the video camera. But if you won't need to enlarge the still much at all, and if quality isn't a major concern, you can always export stills from the video footage after you've recorded and captured the footage.

Canon S3IS

Try the Canon S3IS, it is the perfct choice for you. Excelent video quality and 6 megapixels images. And it does just that. It takes still images while recording video.
 
You could always make an audible or visible note, and then take a still from the video later on, as many cameras use the same sensor anyway.

...if, of course, a separate still camera isn't an option.
