Update 8/7: It locked up two more times and after more research determined it was the hard drive cable. A quick trip to the local repair shop and $71 later because I'm out of warranty and the problem was solved.
Hardware: mid 2012 13" MacBook Pro with Samsung 840 SSD
1. Earlier today I was running my MBP in clamshell mode attached to an external monitor with Bluetooth keyboard and mouse. Everything was working fine. I disconnected it and went into the living room and when I opened it up it locked up with the beachball. I held the power button to shut it down and on restart it gave me the flashing folder with question mark.
2. Booted off my recovery USB drive and the drive did not show up in disk utility.
3. Reset the PRAM with no luck.
4. Pulled the SSD out, stuck it in an external enclosure, and put the original harddrive back in. Booted off that and the SSD in the enclosure connected fine. Also was able to boot directly off the SSD connected via USB. I was able to verify and repair permissions as well as the disk itself.
5. Put the SSD back in the drive bay and still got the flashing folder on start up.
6. Put the old drive back in, connected the SSD via enclosure, and then booted off the recovery flash drive. Reinstalled 10.8 on it.
7. Reinstalling the OS worked. Popped the drive back in once it was done and it booted up just fine. Lost one of the hard drive mounting screws in the process. No idea where it went.
Any idea what just happened and why it would have suddenly stopped being recognized? Would the manner in which I disconnected my external monitor earlier had anything to do with it? (I don't see why but just tossing it out there.)
